National Disability Insurance Scheme
(NDIS)

The NDIS are dedicated to helping Australians with Disabilities function in our ever growing society as well as they possibly can.
The NDIS provides funding to eligible people with disability to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life.
The NDIS also connects anyone with disability to services in their community, such as us here at Local Community Care.
We can provide support and assistance in things such as Daily Cleaning tasks, Shopping, Social Outings and much more.

My Aged Care
(Home Care Packages - HCP)

My Aged Care is the main entry point into the Aged Care system in Australia. It is a government organisation that is responsible for registrations and assessments relating to Aged Care services.
Home care packages can take anywhere from 3 to 12+ months to be accepted, so while you feel yourself or a loved one does not need assistance straight away, starting the process can be quite beneficial. If at any point the person in question needs more assistance than has been allocated, they can be re-assessed to a level that suits their needs.
The HCP covers things like Domestic Assistance, Lawn Maintenance, Transport to medical appointments, Social support just to name a few.

Baptist Care

BaptistCare are a provider who will either complete your needed services for you, or they will broker out to a service like us here at Local Community Care.

Community Services #1

We work with Community services #1 when it comes to Locals within our are, being recently admitted and discharged from hospital. We use a program known as Community Packages (ComPacks), they facilitate safe and early discharges from hospital by providing eligible patients with a short-term package of care designed to help them regain independence and prevent their readmission to hospital.

Commonwealth Home Support Programme (CHSP)

The CHSP provides entry-level support for older people who need some help to stay at home. Service providers work with them to maintain their independence. Support can include help with daily tasks, home modifications, transport, social support and nursing care.
